Minnesota Delta Epsilon Chi is offering you the opportunity to order embroidered association apparel.

You can personalize your items when ordering for an additional cost in the following ways:

• First Name $5.00
• College Name under design $7.00
• College Name elsewhere on garment $10.00

Designs will be in thread colors selected to best match garment.

Items will be shipped UPS at the following rate:
    1—5 items $7.00     6 items and up $10.00

A shipping surcharge may be applied in special situations — you would be notified prior. Most orders will be shipped within 20 days of receipt. All orders must be prepaid.

Before filling out the Order Form, please make sure you have selected each of the following:

• Logo Design
• Item # (listed with the item)
• Your College (to be printed on the apparel, optional).
